VNA Care is seeking a Licensed Independent Clinical Social Worker (LICSW) or Licensed Certified Social Worker (LCSW) to fill the role of a Home Health Social Worker for the Needham Teams. This position offers a work schedule of 24-32 hours per week between the hours of 7 a.m. to 5 p.m., with no weekends, holidays or evenings required. Clinical supervision is available.
Territory: Patient visits are scheduled in tight geographic areas each day.
Team 1: Westwood, Dedham, Norfolk, Walpole and surrounding towns.
Team 2 : Braintree, Brockton, Canton, Marshfield, Stoughton, Weymouth and surrounding towns.
Qualifications
The ideal candidate will possess a Master of Social Work (MSW) degree and hold a current licensure as an LCSW or LICSW. While experience in visiting patients in a home setting is preferred, it is not mandatory for this role.
Responsibilities
The Home Health LICSW/LCSW will collaborate with an interdisciplinary team to conduct Psychosocial assessments for patients in their homes. They will develop personalized care plans to assist patients and caregivers in accessing community resources, provide short-term counseling support, and facilitate long-term planning in coordination with the patient's Primary Care Physician.
