At Blue Water Home Care & Hospice, we are dedicated to providing compassionate, patient-centered hospice care that brings comfort, dignity, and peace to patients and families. We are seeking a caring and experienced Community Hospice Registered Nurse (RN) to join our interdisciplinary team.
What You'll Do
- Conduct hospice admissions and comprehensive patient assessments.
- Develop, implement, and update individualized plans of care.
- Provide skilled nursing care and symptom management according to physician orders.
- Educate patients, families, and caregivers on disease progression, hospice services, and end-of-life care.
- Coordinate care with physicians, hospitals, skilled nursing facilities, and interdisciplinary team members.
- Assess caregiver needs and provide ongoing support and guidance.
- Supervise LPNs, hospice aides, and paraprofessional staff in accordance with regulatory requirements.
- Complete accurate and timely clinical documentation using RXNT and Enclara.
- Participate in interdisciplinary team meetings, quality improvement initiatives, and on-call coverage.
Qualifications
✔ Current Michigan RN License (unencumbered)
✔ Graduate of an accredited nursing program
✔ Current BLS/CPR Certification
✔ Valid Driver's License, Auto Insurance, and Reliable Transportation
✔ Hospice, Home Health, Palliative Care, or Related Nursing Experience Preferred
✔ Strong assessment, communication, and care coordination skills
