Visiting Nurse Association of Central Jersey Community Health Center (VNACJ CHC) offers comprehensive, community based, preventive, primary care and wellness services for New Jersey residents of all ages at four convenient locations. CHC health care services are designed to meet the specific needs of the entire family – from annual check-ups and immunizations to sick visits, mental health counseling and critical prenatal care.
Position Summary:
We’re seeking a compassionate Family Medicine Physician dedicated to expanding access to high‑quality primary care and strengthening the health of communities across Monmouth County, NJ. As part of our interdisciplinary team, you’ll provide holistic primary care to individuals and families across the lifespan while collaborating with clinicians, nurses, behavioral health providers, and support staff to deliver culturally responsive, patient-centered care. What You’ll Do:
In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $200,000 to USD $296,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits. What You’ll Do:
- Deliver patient-centered, holistic primary care to individuals and families of all ages within established productivity and quality guidelines
- Maintain accurate, timely documentation in the Electronic Medical Record
- Ensure compliance with NJ Department of Health regulations and other accrediting bodies
- Participate in team-based care within the Patient-Centered Medical Home (PCMH) model
- Engage in peer review, quality improvement initiatives, and performance enhancement activities
- Provide in-service education and collaborate with staff development efforts
- Support community outreach programs and represent CHC services in the community
- Uphold infection control, safety standards, and organizational policies
- Provide supervision to clinical staff as appropriate
- Contribute to a positive, mission-driven workplace culture focused on respect, collaboration, and excellence
- Licensed Physician in the State of New Jersey
- Board Certified in Family Medicine
- Experience in primary care; FQHC experience preferred
- Active CDS and DEA license
- Bilingual (Spanish/English) strongly preferred
- Valid NJ driver’s license and access to a vehicle
