Home Health Social Worker (Contingent)

Blue Water Home Care is currently seeking a compassionate and motivated Home Health Social Worker to join our interdisciplinary care team on a contingent basis.
Position Summary
The Home Health Social Worker provides supportive services to patients and their families receiving home health care. This role focuses on assessing social, emotional, and environmental factors that impact patient care and recovery. The Social Worker collaborates closely with clinical staff and serves as a link to community resources to support safe, effective care in the home setting.
Key Responsibilities
- Assess the social, emotional, and psychosocial needs of patients and families in the home health program
- Prepare social histories and provide social casework in accordance with agency policies and regulatory standards
- Collaborate with nurses, therapists, physicians, and other members of the interdisciplinary team to support patient-centered care
- Identify and coordinate community resources to assist patients with financial, housing, transportation, and support needs
- Provide counseling, education, and advocacy to patients and caregivers
- Serve as a resource to home health staff regarding psychosocial and environmental concerns impacting patient outcomes
Qualifications
- Bachelor’s degree in Social Work from a Council on Social Work Education (CSWE)–accredited institution required
- Master of Social Work (MSW) from a CSWE-accredited program and State of Michigan Licensed Master Social Worker (LMSW) preferred
- Minimum of one (1) year of social work experience in a healthcare setting (home health experience preferred)
- Valid driver’s license and reliable transportation required
