We are looking for a talented contributor with a deep commitment to enhancing the quality of life for seniors who can manage all aspects of sales and marketing, while building and maintaining positive relationships with prospects and referral sources.
As a Community Liaison, you will be responsible for building and maintaining referral relationships with hospitals, hospices, home health, assisted living communities, physicians, and other healthcare partners. Your primary objective will be to drive topline revenue through increased community awareness of our services.
- Develop and maintain referral relationships with healthcare professionals, case managers, discharge planners, and community organizations.
- Represent the agency in the community through scheduled visits, networking, outreach events, and local health fairs.
- Increase billed hours, develop profitable referral source pipeline, and gain market share.
- Provide education on agency services, intake process, and care capabilities.
- Work collaboratively with the care team to ensure smooth patient transitions.
- Track outreach activities and maintain CRM.
- Maintain compliance with HIPAA, privacy rules, and company policies.
- Monday to Friday, combination of in-office and regular territory visits
- Occasional evenings/weekends for events and responding to inbound sales inquiries
- 2 years Sales experience in home healthcare, hospice, senior care, or community outreach (preferred)
- Strong verbal, written, and interpersonal communication skills
- Self-starter with the ability to manage time and priorities independently
- Reliable vehicle, valid California driver's license, and proof of auto insurance
- Familiarity with general healthcare and senior care landscape is a plus
- Must be legally authorized to work in the United States
