At Visiting Angels of East Central Indiana, we help older adults remain safe, independent, and connected to the comfort of home.
We are looking for a Care Manager who can be the bridge between our clients, families, caregivers, and office team.
This is not just a desk job.
This is a leadership role for someone who can:
- Build trust with families
- Support and coach caregivers
- Notice changes in client needs
- Help solve problems before they become bigger issues
- Keep care plans, communication, and documentation organized and compliant
What You’ll Do
- Conduct client assessments, supervisory visits, and caregiver observations
- Support care plan updates and service changes
- Follow up with families and address concerns
- Coach caregivers in the field and provide encouragement and feedback
- Monitor for changes in condition, safety concerns, falls, dementia-related needs, or caregiver issues
- Document accurately in WellSky and maintain compliance
- Partner with scheduling, intake, training, and office staff to ensure clients receive the right care from the right caregiver
What We’re Looking For
- Experience in home care, healthcare, senior care, case management, social work, or caregiving
- Strong communication, organization, and problem-solving skills
- Someone calm under pressure who can handle sensitive situations with compassion and professionalism
- Experience with dementia, caregiver coaching, care coordination, or supervisory visits is a plus
- Reliable transportation and willingness to travel throughout our service area
