Role Description The Client Case Manager is a full-time, on-site role based in Simpsonville, KY, responsible for coordinating and overseeing client care services. This position manages client caseloads by conducting initial assessments, developing individualized care plans, and performing regular follow-up visits or check-ins to ensure services meet evolving needs. The Client Case Manager works closely with caregivers to match them with clients, communicate care expectations, and support them in delivering safe, compassionate, and reliable care. Daily tasks include updating care documentation, maintaining accurate records in agency systems, and collaborating with families, healthcare professionals, and internal staff to resolve concerns promptly. The role also involves participating in scheduling support as needed, assisting with caregiver orientation or training, and contributing to quality improvement efforts to enhance client satisfaction and outcomes.

Qualifications

  • Client service and communication skills, including the ability to build rapport with older adults, families, and caregivers, and handle sensitive conversations with empathy and professionalism.
  • Organizational and case management abilities, such as managing multiple client cases, prioritizing tasks, and maintaining accurate, timely documentation.
  • Problem-solving and critical-thinking skills to identify changing client needs, address concerns, and coordinate appropriate adjustments to care plans.
  • Basic administrative and computer proficiency, including comfort with scheduling, electronic record-keeping, and standard office software.
  • Knowledge of or interest in home care, senior care, or healthcare settings; prior experience in care coordination, social services, or a related field is preferred.
  • Strong teamwork skills and a collaborative approach to working with caregivers, office staff, and external partners.
  • High school diploma or equivalent required; college education preferred; additional education in social work, nursing, healthcare administration, or a related field is a plus.
  • Sales and Marketing experience is a plus
  • Reliable transportation and the ability to travel locally for client visits and meetings as needed.
