Responsibilities
- Provide attentive patient observation to monitor health status and respond promptly to changes.
- Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
- Administer medications accurately following medical documentation and HIPAA regulations to ensure patient safety.
- Support clients with memory care needs such as Alzheimer's and dementia, utilizing specialized techniques for safety and comfort.
- Operate electronic health record (EHR) or electronic medical record (EMR) systems to document care activities and vital signs meticulously.
- Provide Hospice support as needed, maintaining dignity and comfort for clients.
- Use assistive devices like Hoyer lifts safely during transfers, ensuring proper technique to prevent injury.
- Prepare nutritious meals aligned with dietary restrictions and assist with laundry and light cleaning to maintain a clean living environment.
- Conduct regular vital signs checks and assist with medical procedures such as medication administration under supervision or guidance.
- Maintain detailed medical records in compliance with HIPAA standards while ensuring confidentiality at all times.
- Support clients’ emotional well-being through compassionate communication and engaging activities tailored to individual needs.
Skills
- Proven experience in assisted living, home health, or long-term care environments with a focus on senior or disability care.
- Strong knowledge of medical terminology, patient care protocols, and documentation practices using EHR/EMR systems.
- Skilled in assisting with ADLs, including bathing, dressing, mobility assistance, and catheter or wound care.
- Familiarity with dementia and Alzheimer’s care techniques to promote safety and comfort for memory-impaired individuals.
- Ability to monitor vital signs accurately and recognize signs of medical emergencies requiring immediate attention.
- Experience operating mobility aids such as Hoyer lifts safely while maintaining client dignity.
- Excellent communication skills for working effectively with clients, families, healthcare teams, and other caregivers.
- Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times.
