Deliver Comprehensive Senior Care in a Collaborative Setting
As a Nurse Practitioner/Primary Care Provider, you will perform patient assessments, develop care plans, coordinate care, and contribute to quality improvement initiatives.
Responsibilities
- Patient Assessments: Perform comprehensive assessments, including initial, semi-annual, and unscheduled visits.
- Care Planning: Collaborate with physicians and the interdisciplinary team (IDT) to develop individualized care plans.
- Discharge Planning: Work with physicians and the IDT to facilitate smooth transitions from care settings.
- Nursing Home Care: Maintain and oversee care for participants in nursing homes.
- Care Coordination: Collaborate with providers and the IDT to ensure effective care coordination.
- Quality Improvement: Review data to identify areas for improvement and contribute to best practices.
- On-Call Coverage: Participate in a rotating on-call schedule.
- Education: Graduation from an accredited School of Nursing.
- Certification: Certified as a Nurse Practitioner with Prescriptive Authority.
- Licensure: Active Nurse Practitioner State License.
- Experience: Minimum of two years of experience working with the senior population.
- Knowledge: Current knowledge of chronic care/geriatric issues and best practices.
- Other: Valid State Driver's License with a clean DMV record and proof of vehicle insurance.
