Job Summary And Responsibilities

As our Behavioral Health Coordinator, you will play a crucial role in ensuring patients and their families feel supported and informed throughout their clinic experience. You will collaborate effectively with a multidisciplinary team to facilitate the delivery of impactful and comprehensive treatment plans for our patients.

To be successful in this role, you must possess exceptional communication and interpersonal skills to build trust and rapport with patients and their families. This foundation will be crucial for creating a supportive environment where patients feel heard and understood.

  • Plans and implements an overall 12-month cycle educational curriculum for the Family Medicine Residency Program in compliance with the American Board of Family Medicine and Accreditation Council for Graduate Medical Education (ACGME) guidelines and related accreditation/certification and academic/professional standards; incorporates the integration of the milestones and entrustable professional activities into the curriculum as dictated by specific family/behavioral health requirements.
  • Interviews all applicants (approximately 80 annually) for family medicine residency training positions and develops/provides an applicant assessment to the Program Director.
  • Provides didactic resident training in behavioral medicine, including, but not limited to: counseling, stress reduction, biofeedback, family advocacy and domestic abuse, communication and organization development; provides behavioral science orientations and other education as requested by the Residency Program Director.
  • Implements 360-degree evaluation system to identify, track and evaluate the development of resident skills in accordance with ACGME goals; ensures the inclusion of dependable measures for assessing residents’ competence.
  • Performs patient assessment/data collection, interprets findings and formulates treatment plans; completes all required documentation in accordance with CHI-FH standards and accreditation/regulatory requirements.
  • Implements and updates psychosocial plan of care and treatment based on patient needs throughout patient’s course of care; serves as liaison between providers and community outpatient mental health resources.

Job Requirements

Required

  • Masters Other in Social Work and six years of related work experience that would demonstrate attainment of the requisite job knowledge/abilities
  • Licensed Independent Clinical Social Worker: WA, or
  • Licensed Advanced Social Worker: WA

Preferred

  • Experience in an ACGME-accredited program

Pay Range

$41.15 - $67.45 /hour
