Role Description
This is a contract role for an Admission Nurse, based in Fredericksburg, VA, with a hybrid work arrangement that allows for some work-from-home flexibility. Responsibilities include conducting comprehensive client assessments, coordinating admission processes, developing individualized care plans, managing patient communications, and ensuring compliance with healthcare standards. The Admission Nurse will also collaborate with the interdisciplinary care team to provide seamless transitions and ongoing support.
Qualifications
- Expertise in Nursing, Medicine, and Home Care
- Experience in Palliative Care and Long-term Care
- Strong organizational, communication, and interpersonal skills
- Ability to work independently and collaboratively within a hybrid setting
- Active Registered Nurse (RN) license in the state of Virginia
- CPR certification and knowledge of healthcare regulations
- Prior experience in home healthcare or admission assessments is a plus
