We are seeking a proven leader who can think strategically, solve problems confidently, develop others, and strengthen the partnerships of our Care Team, Clients, and their Families.
For more than 13 years, VHCP has served Clients and Families throughout Fredericksburg and the surrounding communities. As a locally owned, independent home care agency, we have built our reputation by fostering personal relationships called partnerships, being consistent, professional, and sharing a genuine commitment to our mission:

This individual approaches scheduling strategically—considering the needs of Clients and Care Partners and the long-term stability of each partnership.
The right person will lead with a balance of warmth and competence—helping people feel heard, respected, and supported while providing clear direction, exercising sound judgment, and maintaining high personal and professional standards. This leader will remain composed when plans change, take responsibility for finding solutions, and recognize opportunities to strengthen both people and processes.
The Office Coordinator will consistently model our core value:
Lead with honor and respect, and receive honor and respect.
Key Responsibilities
- Coordinate Client schedules with accuracy, foresight, and a strong understanding of each Client’s needs and preferences.
- Approach scheduling as a strategic responsibility and develop dependable solutions.
- Engage in effective communication via phone calls and email.
- Take an on-call rotation and respond effectively to communication, schedule changes, service concerns, and other time-sensitive needs.
- Promote consistency of care by thoughtfully matching Care Partners with Clients in coordination with Care Management.
- Serve as a leader within Care Management, contributing sound judgment, clear communication, accountability, and follow-through.
- Lead with warmth and competence, demonstrating genuine care for people while confidently upholding policies, expectations, and professional standards.
- Build trusted partnerships with Care Team Partners, Clients, families, nurses, and fellow Care Management team members.
- Provide Care Partners with the guidance, encouragement, expectations, and support they need to succeed.
- Process Care Team Partner records
- Invest in the development of others by recognizing strengths, offering constructive feedback, and creating growth opportunities.
- Address concerns directly, respectfully, and promptly while protecting each person’s dignity.
- Communicate scheduling updates and service information clearly and professionally.
- Identify recurring challenges and recommend improvements to systems, communication, and workflow.
- Serve as support for Care Management
