Job Duties
Social Workers provide social services programs in order to provide for the needs of the residents of Sitter and Barfoot Veterans Care Center. They are responsible for helping to enhance residents’ quality of life by providing residents, their families, and other groups with psychosocial support. Our social workers are responsible for their individual units, while working as part of the social work team and as part of other diverse and interdisciplinary teams as needed.

Job Requirements:
- Plans, directs, and coordinates comprehensive clinical social work services.
- Identifies and evaluates personal, emotional, and environmental concerns to meet psychosocial needs of residents.
- Identify social, psychological, physical, and/or environmental impediments to resident progress through interviews and resident records.
- Complete Resident Full Assessment Form and Progress Notes by the Assessment Reference Date (ARD) and place in resident’s chart.
- Complete reassessment of all residents at regular intervals or as warranted by changes in condition and document findings.
- Develop and maintain close working relationships with both public and private health organizations and community resources.
- Work with the IDCP team to develop a care plan individually designed to assist the resident in attaining the highest possible physical, mental and psychosocial functioning.
- Assist IDCP team in the coordination of discharge planning.
- Bachelor’s degree in social services, social work, or related field.
- Excellent knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.
- Demonstrated knowledge of principles and methods of providing social services.
- Knowledge of State and Federal regulations, policies, and procedures related to the field of social work and long-term care.
- Knowledge of administrative and clerical procedures and systems, such as word processing, managing files and records, designing forms, and other office procedures and terminology.
- Excellent knowledge of principles and processes for providing customer service.
- Basic knowledge of computer software such as Microsoft Office.
- Ability to effectively communicate both written and orally.
- A Master’s degree in Social Work is preferred.
- 1 Year prior social work experience preferred.
- Prior social work experience in a Long-Term Care (LTC) facility is preferred.
