What are the responsibilities and job description for the Discharge Planner - Care Management - Days position at Virginia Commonwealth University Health Systems?
The Discharge Planner is proficient with Medicare, Medicaid, and Commercial Insurance rules/regulations for nursing facility level and skilled level of care for placement/post hospitalization. This job requires knowledge of appropriate resources (community) for post hospitalization care. This job requires a critical thinker able to work independently while interacting with multiple Healthcenter individuals. This job requires superior people skills in stressful situations. Experience PREFERRED Two (2 ) or more years of experience in acute care or other healthcare setting with emphasis on care management/discharge planning

Education/training PREFERRED: Bachelor's Degree in Social Work from an accredited program
