Position Summary
The Registered Nurse (RN) is responsible for providing high-quality, compassionate nursing care in accordance with the Connecticut Board of Nursing standards, clinical best practices, and facility policies. The RN coordinates and delivers patient-centered care, promotes wellness, and ensures continuity of treatment through effective communication, assessment, and clinical leadership.
Key Responsibilities
Perform comprehensive nursing assessments and develop individualized care plans based on patient needs.
Administer medications, treatments, and procedures as prescribed by the physician.
Monitor and document patient progress, response to care, and any changes in condition.
Collaborate with physicians, LPNs, CNAs, therapists, and other interdisciplinary team members to ensure coordinated care.
Provide patient and family education regarding diagnoses, treatment plans, and health maintenance.
Supervise and mentor LPNs, CNAs, and other nursing support staff.
Maintain accurate and timely electronic health records (EHR) and clinical documentation.
Ensure adherence to infection control, HIPAA, OSHA, and Connecticut Department of Public Health regulations.
Participate in care conferences, audits, and performance improvement initiatives.
Respond to medical emergencies and execute sound clinical judgment in high-pressure situations.
Qualifications
Active and unrestricted Connecticut RN license.
Graduate of an accredited nursing program (Associate or Bachelor’s degree in Nursing required; BSN preferred).
Current CPR/BLS certification (ACLS preferred).
Minimum 1–2 years of clinical experience (experience in home health, long-term care, or acute care preferred).
Strong clinical assessment, critical thinking, and communication skills.
Ability to multitask, prioritize, and adapt in a fast-paced environment.
Must be available a minimum of 3 days per week (including potential weekends or holidays as needed).
Preferred Skills
Proficiency with electronic health record (EHR) systems (e.g., Epic, PointClickCare, HomeCare HomeBase).
Experience with wound care, IV therapy, and medication management.
Leadership and team coordination experience in a clinical setting.
