Overview
We are seeking a dedicated Direct Care Worker (DCW) to join our team and deliver exceptional care to individuals with diverse needs. This role offers an exciting opportunity to make a meaningful difference in the lives of clients by assisting with daily activities, health monitoring, and providing compassionate support in various care settings.
Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and record vital signs such as blood pressure, pulse, and temperature to ensure health stability
- Support medication administration following prescribed guidelines while maintaining HIPAA compliance for patient privacy
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
- Use specialized equipment such as Hoyer lifts or resident’s
to ensure safe transfers and mobility assistance - Observe and document patient conditions, behaviors, and changes to inform care plans and communicate effectively with healthcare teams
- Maintain a clean, safe environment by performing light housekeeping tasks and adhering to infection control protocols
Requirements
- Prior experience in group homes, assisted living facilities, nursing homes, or home & community care environments is highly preferred
- Knowledge of patient observation techniques, Alzheimer’s care, dementia care, memory care, autism support, hospice care, and working with individuals with disabilities or developmental challenges
- Certification in CPR and first aid is required; additional certifications such as HIPAA training are a plus
- Ability to lift heavy objects safely and perform physical tasks involving mobility assistance or transfers
- Strong communication skills for working collaboratively with clients, families, and healthcare professionals
- Experience with medication administration, vital signs monitoring, and developing or following care plans is desirable
- Compassionate attitude with a commitment to providing respectful, person-centered care in home health or long-term care settings
