Job Summary
As the Community Nurse it your responsibility to assist the Health Care Coordinator with the coordination of health care for our residents. You will assist with recruiting, interviewing, hiring, training, scheduling and supervision of Universal Workers. You will be responsible for maintaining accurate and timely documentation as related to your position in accordance with Vintage Park policies and Local, State and/or Federal regulations. You will assist the Health Care Coordinator with timely assessments and re-assessments of prospective and current residents; oversight of the provision of personal cares and medication administration for residents. This position requires being on-call and filling in as required.
Duties
- DUTIES AND RESPONSIBILITIES: RESIDENT HEALTH CARE:
Assist Health Care Coordinator with completing any and all assessments, along with development of the ISP and related follow ups, as required for proper resident care and Assisted Living Regulations and/or Vintage Park policies and procedures.
Assist the Health Care Coordinator with timely and accurate medication administration as required (monitoring medications for residents who receive medication management, prepare and update EMARS and ensure the accuracy of the EMARS, ensure medications reminders and administration is accurate and up-to-date, process any resident medication changes accurately and in a timely manner).
Ensure incident reports are generated when necessary, review and follow up as needed.
Notify the appropriate medical professionals and/or family members of resident health concerns as directed, follow up as necessary.
Receive and chart physicians’ orders as needed. Monitor that the orders have been followed and documented accurately and in a timely manner.
Document resident’s health concerns in Nurse’s notes or progress notes as required.
Communicate with UW’s/staff re: resident health concerns requiring staff/UW’s attention and/or follow up during their shifts.
Assist with administering TB tests, employee drug screens, flu shots, etc., as directed.
Monitor PCC dashboard regularly and follow up as needed to ensure timely and accurate provision of service to our residents.
Maintain confidentiality as per HIPAA regulations.
Experience
- Valid LPN or RN licensure with current state certification required; additional certifications such as Basic Life Support (BLS) preferred.
- Assist with providing 24-hour, 7 days per week on-site coverage and on-call duties as assigned
Respond to calls for urgent assistance from the Vintage Park Community
Respond to fire alarms and other emergencies as directed.
VINTAGE PARK STAFF:
Assist the Manager and/or Health Care Coordinator with recruiting, hiring, training, scheduling and supervision of Universal Workers as directed.
Ensure that Vintage Park staff are aware of and responding to the changing health needs of our residents.
OTHER:
Participate in Interdisciplinary Team Meetings as directed (Safety Committee, Mandatory Staff Meetings, Stand-up Meetings, in-services trainings, etc.).
Ensure the proper disposal of medications and hazardous materials.
Obtain and maintain a valid Iowa chauffer’s license within 90 days of hire.
Be aware of and comply with all state and federal regulations governing assisted living facilities and Vintage Park Policies and Procedures.
Follow budget guidelines—maintain inventory of nursing supplies and equipment and monitor usage as necessary.
Contribute to a positive and productive work environment.
Work cooperatively with your supervisor and fellow staff members.
Other duties as assigned by the manager or his/her designee.
REQUIRED SKILLS AND ABILITIES:
Able to support the mission of Vintage Park Apartments
Good communication (articulation and listening), problem solving, analytical and organizational skills
Able to prioritize job duties and manage several tasks simultaneously
Familiar with the use of a tablet, lap top computer, smart phone, walkie-talkies, and other electronic devices
PHYSICAL REQUIREMENTS:
Able to sit or stand for long periods of time
Able to assist with lifting
Able to complete visual checks and observation of staff and residents
REQUIRED EDUCATION AND/OR TRAINING:
Maintain current Iowa RN or LPN License
Able to pass work-related drug screenings and all required background checks
Complete all training requirements at Vintage Park in a timely fashion.
Pay: $30.00 - $35.00 per hour
