Responsibilities
- Assist with patient care activities such as vital signs monitoring, medication administration, and basic nursing procedures including catheter care and tube feeding.
- Support residents and patients with activities of daily living in assisted living, memory care, hospice, or home care settings.
- Maintain accurate medical documentation using Electronic Medical Record (EMR) systems like Epic or Cerner while adhering to HIPAA regulations to protect patient privacy.
- Collaborate with nursing staff and healthcare teams to implement care plans tailored to individuals with developmental disabilities, Alzheimer’s disease, dementia, or other cognitive impairments.
- Conduct routine assessments of patients’ physiological status by measuring vital signs and observing physical condition changes.
- Assist with medical procedures requiring knowledge of anatomy and physiology, ensuring safety and comfort during treatments such as medication administration or catheter care.
- Document all patient interactions thoroughly in medical records and communicate effectively with team members regarding patient status or changes in condition.
Qualifications
- Prior experience working in assisted living facilities, nursing homes, hospitals, or home & community care environments is highly preferred.
- Knowledge of medical terminology, anatomy, physiology, and clinical procedures such as tube feeding and catheter care.
- Familiarity with EMR systems like Epic or Cerner; experience with EHR systems is a plus.
- Strong understanding of HIPAA regulations related to medical documentation and patient privacy.
- Demonstrated caregiving skills for individuals with disabilities or cognitive impairments such as Alzheimer’s or dementia.
- Ability to work effectively in fast-paced settings including acute care environments and hospice situations.
