A Brief Overview

Core responsibilities include conducting psychosocial assessments, coordinating care, providing counselling and psychotherapy, intervening in client crisis situations, and educating patients and families about their treatment plan and the available resources and support systems. Works collaboratively as a part of the multidisciplinary team to provide palliative education and support to patients, families, and members of the healthcare team

What you will do

  • Assist the Bereavement Counselor in providing emotional support and guidance to grieving individuals and families.
  • Conduct initial intake interviews to assess the needs of clients and schedule appointments.
  • Maintain regular contact with clients through phone calls, emails, and follow-up visits to ensure they receive ongoing support.
  • Manage client records and ensure all documentation is accurate and up-to-date.
  • Coordinate and schedule support group meetings, workshops, and other bereavement-related events.
  • Prepare and distribute informational materials and resources to clients and their families.
  • Assist in the planning and implementation of bereavement support programs and activities.
  • Collaborate with community organizations, volunteers, and other support services to enhance program offerings.
  • Monitor and evaluate the effectiveness of bereavement support programs and provide feedback for improvements.
  • Serve as a point of contact for clients, families, and external partners regarding bereavement services.
  • Facilitate effective communication between the Bereavement Counselor and clients.
  • Promote awareness of bereavement services through outreach efforts and community engagement.
  • Performs intra and interpersonal counseling services with individuals and groups regarding grief counseling and co-occurring diagnoses.
  • Develop and implement individualized treatment plans based on client needs and therapeutic goals.
  • Provide evidence-based therapeutic interventions, including individual, group, and family therapy.
  • Assists with all other duties as assigned.

Education Qualifications

  • Master's Degree in Social Work (MSW) preferred. Degree in psychology, social work, counseling, or a related field (or equivalent experience) is preferred.

Experience Qualifications

  • Previous experience in a support or counseling role (preferred but not required).

Knowledge, Skills, and Abilities

  • Advanced social work assessment skills, communicate effectively, critical thinking skills. Able to collaborate as part of a multidisciplinary team

Licenses and Certifications

  • Grief Certification preferred

Pay: $74,000.00 - $76,000.00 per year
