Mobile Wound Care Advanced Practice Provider (NP/PA) – Field-Based

Position Summary

Victory Wound Care is seeking a dependable, detail-oriented, and clinically strong Advanced Practice Provider to provide mobile wound care throughout the Greater Austin and Central Texas area.

This is a field-based position, not a clinic or remote role. Providers travel directly to patient homes, assisted living communities, independent living communities, and other residential settings throughout the service area. A significant portion of the workday may involve driving between patients.Success in this position requires excellent clinical judgment, time management, documentation accuracy, responsiveness, and the ability to work independently while remaining closely connected with the broader clinical and administrative team.Prior wound care experience is preferred but is not the only factor we consider. We place significant value on providers who are dependable, coachable, collaborative, responsive, detail-oriented, and willing to continuously develop their wound care knowledge and clinical practice.

Key Responsibilities

• Perform comprehensive wound evaluations, including wound assessment, medical history review, identification of contributing factors, and evaluation of healing progress.
• Develop and update individualized wound care plans for patients with chronic and complex wounds, including pressure injuries, diabetic ulcers, venous ulcers, arterial wounds, traumatic wounds, and other difficult-to-heal wounds.
• Determine appropriate treatment strategies based on wound presentation, patient comorbidities, vascular status, goals of care, and current evidence-based practices.
• Perform or oversee wound care procedures within scope of practice, including debridement and other advanced wound treatments when clinically appropriate.
• Evaluate patients for advanced wound care therapies and determine medical necessity in accordance with clinical and payer requirements.
• Review wound measurements, photographs, treatment response, and other clinical information to determine whether the current plan of care remains appropriate.
• Coordinate care with nurses, home health agencies, primary care providers, specialists, facilities, caregivers, and other members of the patient’s healthcare team.
• Provide clear wound care orders and communicate changes in the plan of care to appropriate partners.
• Educate patients and caregivers regarding wound care, pressure relief, compression, nutrition, infection prevention, and other factors affecting wound healing.
• Identify clinical changes that require escalation, referral, additional testing, emergency evaluation, or a higher level of care.
• Review clinical documentation completed by supporting nursing staff when applicable and provide timely feedback or clarification when corrections are needed.
• Maintain accurate, complete, and clinically defensible documentation that supports the patient’s condition, treatment plan, procedures performed, and medical necessity.
• Complete all clinical documentation within required timeframes so that care coordination, billing, home health communication, and other downstream processes are not delayed.
• Participate in utilization review, documentation review, quality improvement, and ongoing clinical education.
• Incorporate supervisory, peer, and documentation feedback into clinical practice.
• Remain current on wound care standards, payer requirements, and evidence-based treatment approaches.

Mobile Scheduling and Documentation Expectations

Victory Wound Care provides flexibility in how providers structure their schedules; however, that flexibility requires strong personal accountability and time management.Providers are responsible for planning appropriately for patient visit duration, geographic routing, and travel time between appointments. Patient availability and clinical needs may occasionally cause schedules to change throughout the day.Some days may require documentation or follow-up responsibilities to be completed after 5:00 p.m. Clinical documentation is expected to be completed within established charting deadlines, generally within 24 hours of the visit, so that care coordination and communication with home health agencies and other members of the healthcare team can occur without unnecessary delay.Timely communication is equally important. Providers are expected to remain reasonably responsive to clinical and operational communication throughout the workweek and to promptly escalate questions or concerns when guidance is needed.

Qualifications

• Current and unrestricted Texas licensure as an advanced practice provider, including Nurse Practitioner or Physician Assistant.
• Appropriate prescribing authority and credentialing as required for the position.
• Ability to reliably travel throughout the Greater Austin and Central Texas service area.
• Comfortable providing care independently in patient homes and residential healthcare settings.
• Strong clinical assessment, differential diagnosis, and treatment-planning skills.
• Excellent organizational and time-management abilities.
• Strong written and verbal communication skills.
• Ability to produce thorough, accurate, and timely clinical documentation within an electronic health record.
• Comfortable using mobile technology and electronic communication platforms.
• Ability to collaborate effectively with nurses, home health agencies, physicians, specialists, caregivers, and administrative staff.
• Receptive to clinical, documentation, and performance feedback.
• Willingness to participate in ongoing wound care education and professional development.

Physical Requirements

• Ability to travel frequently by vehicle throughout the assigned service area.
• Ability to transport routine clinical supplies and equipment.
• Ability to perform patient assessments and wound care that may require bending, kneeling, or working near floor level.
• Ability to safely perform procedures and patient care activities required within the provider’s scope of practice.

Compensation

This is a full-time contract position utilizing a per-patient-visit compensation structure.Depending on availability, patient volume, experience, and scheduling, compensation is expected to be equivalent to approximately $135,000–$160,000 annually. At this time, the position does not include employer-sponsored health insurance or traditional employee benefits. Compensation is structured accordingly for an independent contractor arrangement.

Pay: $135,000.00 - $160,000.00 per year
