Over Twenty years ago, our founders created a different kind of human services agency: one that provides high quality services to communities of color that have been historically disadvantaged, while also being at the forefront of advocating for the rights of people with disabilities. They were frustrated with the lack of leadership and career opportunities for Blacks, Indigenous, People of Color (BIPOC). Even though many human services professionals are people of color, there were few in leadership and change-making positions. Our founders took the bold step of creating a new agency.
Today, we are the largest minority-led human service agency in Massachusetts, our leadership team and Board are predominately comprised of people of color. We provide in-home care and support to seniors and to individuals with developmental disabilities in the Boston, Brockton, and Springfield areas.

Overview of Transitional Assistance Coordinator Position.
We are seeking a dedicated and skilled Transitional Assistance Coordinator to join our team. The TA Coordinator will be responsible for managing client transitions, developing transition plans, and collaborating with internal and external partners to ensure smooth community transitions for our clients. The ideal candidate will possess strong organizational, communication, and problem-solving skills, along with a background in TA and hosing search expertise.
Qualifications
Professional Characteristics
· Bachelor’s degree in related fields or associate degree with a minimum of 2 years of TA experience.
· Proficiently use MS Excel, Word, PowerPoint, and Office 365.
· Bring 2 years of TA experience, including housing search experience.
· Showcase good budgeting skills.
Personal Characteristics
· Must have very good verbal and writing skills.
· Must be skilled in use of MS Word
· Must have excellent work habits
· Self-motivation, organized and flexible
· Commitment to improve care in underserved populations
· Collaborative work style; relationship building; entails meeting directly with members, their families and providers in home and office settings
· High level of accountability
· Reliable transportation
· Ability to speak multiple languages preferred (i.e. English, Spanish, Portuguese, Creole, Vietnamese)
Key Responsibilities:
1. Client Transition Management
· Provide weekly reports on TA clients to service coordinators and supervisors.
· Manage transitions, including discharge and purchases for TA clients.
· Maintain accurate records of transition activities.
· Complies with State/Federal regulatory acts and agency policies.
2. Adaptability and Communication
· Be flexible and adaptable to changing circumstances.
· Assist TA participants in retrieving core documents during the housing search process.
· Conduct intake for new participants.
3. Transition Planning and Collaboration
· Develop transition plans, timelines, and milestones while addressing concerns and managing expectations.
· Ensure alignment with organizational goals and objectives.
· Coordinate activities, resources, and communication during the transition period.
· Collaborate with team members and external partners for smooth community transitions.
Skills and Requirements
· Demonstrate timely task completion.
· Possess 2 years of experience in voucher applications and housing applications.
· Have reliable transportation for travel within Massachusetts. Able to travel 30-35 miles radius from Brockton and Boston office to see clients.
· Exhibit professional oral and writing skills.
· Multitask effectively.
· Manage 15-20 caseloads concurrently.
· Full-time TA Coordinator is able to bill 160 hours monthly service for clients.
· Part-time TA Coordinator must be able to bill 120 service hours monthly for clients.
· Able to meet deadlines for TA billing and other tasks.
· Must be able to see each client on caseload every three months

· Team Collaboration: The ability to work collaboratively with other team members, service coordinators, supervisors, and external partners is essential to ensure seamless client transitions and support.
· Client Support: The role may involve providing ongoing support to clients during the transition process, addressing their needs, concerns, and barriers to successful integration into the community.
· Compliance and Documentation: Adherence to all relevant regulations, policies, and procedures governing transitional assistance programs, as well as maintaining accurate and up-to-date documentation for reporting and auditing purposes, is required.

· Professional Development: Opportunities for professional growth and development, such as training programs, workshops, or certifications relevant to the field of transitional assistance and social services, may be available.
