Responsibilities
- Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Observe and report on patient conditions, including vital signs and behavioral changes
- Support patients with medication reminders and basic medication administration under supervision
- Help with meal preparation, feeding, and ensuring proper nutrition
- Maintain accurate medical records using EMR/EHR systems in compliance with HIPAA regulations
- Assist with transfers using equipment such as Hoyer lifts and provide safe mobility assistance
- Support patients with dementia, Alzheimer's, memory care, hospice, and long-term care needs
- Perform light housekeeping tasks related to patient care, including laundry and cleaning of care areas
- Document patient interactions accurately and promptly in medical records
- Follow infection control protocols and maintain a clean, safe environment for patients
- Collaborate with healthcare team members to ensure comprehensive patient care
Qualifications
- Currently enrolled or planning to enroll in a certified home health aide training program or similar healthcare education
- Previous experience working with seniors, individuals with developmental disabilities, or in assisted living/nursing home settings is a plus
- Knowledge of medical terminology, HIPAA regulations, and basic medical documentation practices preferred
- Certification or willingness to obtain CPR certification prior to or during employment is required
- Experience working with people with disabilities or special needs is advantageous
- Strong communication skills and compassionate attitude toward patients of all ages and backgrounds
- Ability to work independently as well as part of a multidisciplinary team
- Reliable transportation may be necessary depending on assignment locations
