JOB SUMMARY 

To plan, develop, organize, implement, evaluate, and direct the Social Services Program in accordance with current existing federal, state, and local standards, as well as the established policies and procedures of Vicar's Landing to ensure that the medically related emotional, and social needs of residents are met/maintained on and individual basis. 

 

 

ESSENTIAL DUTIES & RESPONSIBILITIES 

1. Direct and coordinate the admissions process for all prospective residents in the Health center to include completing the admissions packet. 

 

2. Develop, maintain, and annually review departmental policies and procedures to ensure that all current state and federal standards are met. 

 

3. Interpret social, psychological, and emotional needs of the resident and his/her family to the nursing staff, attending physician, and other resident care team members. 

 

4. Complete all initial and ongoing social service documentation to include assessments, evaluations, and progress notes for each resident. 

 

5. Perform marketing functions for the Health Center to include responding to inquiries, conducting facility tours, and staying in daily contact with the appropriate personnel from the local hospitals. 

 

6. Develop an interdepartmental discharge plan for each resident to include available community services which will ensure appropriate support for the resident upon discharge. 

 

7. Function as the person of first contact for residents and family concerning all issues, bringing such issues as may be necessary to the attention of the administrator for resolution. 

 

8. Attend and participate in professional organizations, seminars, workshops, and other activities to ensure ongoing professional development. 

 

9. Compile and track such statistics as may be requested by the administrator to assist in analyzing operations of the Health Center. 

 

10. Serve on various committees of the Health Center as may be requested by the administrator. 

 

11. Plan, organize and coordinate quarterly family council meetings, for residents, family, and staff. 

 

12. Make routine visits to each resident for the purpose of identifying issues which may require staff attention and to update the resident on such issues as residents’ rights. 

 

13. Perform any other related duties as required or assigned. 

Qualifications:

JOB REQUIREMENTS 

Skills, Abilities & Competencies: 

Specific Requirements: 

  • Must possess, as a minimum, a bachelor’s degree in the Social Work from an Accredited College or University. 

  • Must have experience in a supervisory capacity in a hospital, skilled nursing care facility, or other medically related facility. 

  • Must have the ability to make independent decisions when circumstances warrant such action. 

  • Must be knowledgeable of Social Service practices and procedures, as well as all current local, state, and federal regulations and guidelines. 

  • Must have the ability to plan, organize, implement, and interpret program goals and objectives that are necessary for providing quality of care. 

  • Must have patience, tact, cheerful disposition, and enthusiasm, as well as the willingness to handle residents based on whatever maturity level they are currently functioning. 

  • Possess the ability to deal tactfully with personnel, residents, family members, visitors, and the public. 

  • Must be able to read, write, and speak the English language. 

  • Must be able to move intermittently throughout the workday. 

  • Must be able to cope with the mental and emotional stress of the position. 

  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents’ personnel, and support organizations. 

  • Must be able to relate to, and work with, the ill, disabled, and elderly, emotionally upset, and at times hostile people within the facility. 

 

PHYSICAL REQUIREMENTS:  

  • Must be able to move intermittently throughout the workday. 

  • Must be able to speak the English language in an understandable manner. 

  • Must be able to cope with the mental and emotional stress of the position. 

  • Must possess sight/hearing senses, or other prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met. 

  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with other personnel. 

  • Must be in good general health and demonstrate emotional stability. 

  • May be required to lift at least twenty-five (25) pounds, move equipment, and supplies. 

 

PHYSICAL DEMANDS 

While performing the essential functions of this job, the employee is regularly required to stand, walk, sit, talk hear, and occasionally required to reach with hands and arms, stoop, kneel, and crouch.  Employee must have. The employee must occasionally lift and/or move up to 30 pounds.  Specific vision abilities required by the job include close, distance, and peripheral vision, depth perception and the ability to adjust focus as well as work on a computer for extended periods of time.  The employee must be able to communicate verbally with other on the phone or in person.
