POSITION SUMMARY

 

The Physician Assistant (PA) is a member of the interdisciplinary healthcare team and supports the mission and objectives of Via Care Community Health Center. Under the direction of a supervising physician and established written protocols, the PA is administratively responsible to and receives clinical supervision from the Chief Medical Officer. The PA provides high-quality, patient-centered care across multiple clinic locations and community settings, serving as a float provider supporting coverage throughout East Los Angeles (ELA) and on the organization's Mobile Health Unit. Working collaboratively with the care team, the PA delivers primary and preventive care services to diverse patient populations, including individuals with substance use disorders, and should possess experience in Medication-Assisted Treatment (MAT), pediatric care, and bilingual English/Spanish communication.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

 

  • Adhere to all Via Care Community Health Center (VCCHC) policies and procedures.

  • Demonstrate VCCHC's mission, vision, and core values in all interactions.

  • Maintain a positive, professional, and respectful attitude with patients, staff, and community partners.

  • Communicate regularly with the immediate supervisor regarding departmental and organizational concerns.

  • Report to work as scheduled and prepared to perform the duties of the position.

  • Meet established productivity standards and perform duties as workload necessitates.

  • Obtain and document patient medical and social histories, perform physical assessments, and develop appropriate treatment plans.

  • Order, interpret, and review laboratory tests, imaging studies, and other diagnostic procedures in accordance with established protocols and clinical guidelines.

  • Provide preventive care, health education, counseling, primary care services, and chronic disease management.

  • Provide care to patients across the lifespan, including pediatric and adult populations, in accordance with scope of practice.

  • Provide treatment and ongoing management for patients with Substance Use Disorders (SUD), including Medication-Assisted Treatment (MAT), as appropriate.

  • Prescribe and dispense medications in accordance with applicable laws, regulations, and organizational protocols.

  • Consult and collaborate with physicians and other healthcare team members regarding patient care needs and treatment plans.

  • Refer patients to specialty care, behavioral health, social services, and community resources, and ensure appropriate follow-up.

  • Perform pre-visit planning to ensure all necessary laboratory results, imaging reports, consultation notes, and discharge summaries are available prior to patient appointments.

  • Serve as a float provider supporting coverage across East Los Angeles (ELA) clinic sites and provide services through the Mobile Health Unit as assigned.

  • Supervise Medical Assistants, Community Health Workers, and other assigned personnel involved in patient care activities.

  • Participate in case conferences, care management meetings, departmental meetings, team huddles, and other required organizational activities.

  • Ensure patient communications, education, and services are culturally and linguistically appropriate.

  • Collaborate with the care team to address preventive care, care coordination, care management, educational needs, referrals, and follow-up services.

  • Assist with staff training, quality improvement initiatives, data collection, program evaluation, reporting, and research activities as assigned.

  • Complete all required clinical documentation, coding, and billing forms accurately and timely.

  • Support patients and families in care planning, self-management, and navigation of healthcare and community resources.

  • Perform other duties as assigned by the Chief Medical Officer, Medical Director, or designee.

Qualifications:
  • Graduation from an accredited program for Physician’s Assistants.

  • CA licensed as Physician Assistant.

  • Must have MAT and SUD experience

  • Experience providing Medication-Assisted Treatment (MAT) for Substance Use Disorders (SUD)

  • Experience treating SUD patients, including assessment, treatment planning, and ongoing care management

  • Experience providing pediatric care preferred

  • Bilingual in English and Spanish required

 

Additional Requirements:

  • Must have a reliable automobile for use on the job

  • Subject to a criminal background check prior to employment

  • Must possess valid California Driver License

  • Must have and maintain State required automobile insurance coverage

  • TB and post-employment physical clearance, to be renewed every year

Must be able to talk and hear well.  Good vision is imperative.

The following lists physical demands an employee may perform on an occasional basis: lifting an average weight of 20 lbs., crouch/knee/stoop/crawl and reach with hands and arms.

In compliance with California Labor Code § 432.3, as amended effective January 1, 2026, this job posting includes the pay scale for the position. The pay scale represents a good‑faith estimate of the salary or hourly wage range the employer reasonably expects to pay for the position upon hire. Actual compensation within the posted range will depend on experience, skills, and internal equity.

Salary : $140,000 - $160,000
