Job Summary
We are seeking compassionate and dedicated Direct Support Professionals to provide high-quality care and assistance to individuals with developmental disabilities, aging needs, or other health challenges. In this role, you will support residents in their daily activities, promote independence, and ensure a safe and nurturing environment. This role is for a sponsored residential provider in which you care for individuals with ID/DD in your home!
Responsibilities
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor residents’ health status by observing patient conditions
- Administer medications following care plans and HIPAA guidelines
- Prepare nutritious meals and assist with meal planning in accordance with dietary requirements
- Support residents during recreational activities, social outings, and community engagement
- Maintain cleanliness of living spaces through routine cleaning and sanitation tasks
- Operate equipment such as Hoyer lifts and assist with transfers safely and effectively - client dependent
- Document care activities accurately in compliance with healthcare regulations and facility policies
- Follow safety protocols including CPR, first aid, and medication administration
- Collaborate with healthcare professionals, family members, and team members to develop and implement individualized care plans
Skills
- Proven experience in assisted living, home & community care, or long-term care environments
- Knowledge of patient care techniques including medication administration, vital signs monitoring, and patient observation
- Familiarity with specialized care such as Alzheimer’s/dementia/memory care and autism support
- Ability to perform heavy lifting safely when required for resident transfers or mobility assistance
- Strong communication skills to effectively interact with residents, families, and team members
- Certification in CPR and First Aid is preferred; ongoing training in HIPAA compliance is essential
- Compassionate attitude with patience for working with individuals with disabilities or complex health needs
- Ability to work independently as well as part of a multidisciplinary team in various settings including nursing homes or home health environments

- Applicants would be required to have six months of experience in Direct care with individuals with ID/DD. Applicants would also have to complete the application process which includes but is not limited to: Federal/state back ground check, 90-day proof of finances; Driver's license; Live within 100 miles of Altavista VA.
