Director of Care Navigation - Vetter Senior Living

Vetter Senior Living, a premier provider of senior living and healthcare services, is seeking a passionate and experienced Director of Care Navigation to support our communities in Omaha, Papillion, Elkhorn, Wahoo, and Lincoln.

This role is instrumental in connecting individuals to the right level of care at the right time, using clinical insight, system tools, and collaboration to guide placement into the most appropriate Vetter community or service line.

Key Responsibilities

• Provide leadership and oversight for care navigation processes across multiple communities
• Utilize electronic referral systems to review, prioritize, and respond to incoming referrals in a timely manner
• Analyze clinical, financial, and operational data within software systems to determine the most appropriate placement for each individual (rehabilitation, skilled nursing, or other services)
• Partner with hospitals, physicians, discharge planners, and case managers to coordinate smooth and efficient transitions of care
• Guide and support admissions teams in using referral technology to optimize acceptance decisions, response times, and conversion rates
• Ensure accurate documentation, communication, and follow-up within referral and EMR systems to support continuity of care
• Collaborate with facility leadership to align placements with clinical capabilities, census goals, and resident needs
• Identify trends within referral data and software systems to improve processes, enhance outcomes, and drive growth
• Build strong relationships with referral sources by demonstrating responsiveness, professionalism, and expertise in navigating care options
• Travel periodically between assigned locations to support teams and strengthen relationships

Qualifications

• Licensed nurse (RN preferred) or equivalent healthcare leadership experience
• Minimum 3--5 years of experience in post-acute care, senior living, admissions, or care coordination
• Experience using EMR systems, hospital referral platforms, and admissions/CRM tools
• Strong clinical judgment with the ability to match patient needs to appropriate levels of care
• Excellent communication, organization, and relationship-building skills
• Ability to manage multiple priorities and make timely, data-informed decisions
• Willingness to travel within Nebraska
