What are the responsibilities and job description for the Social Worker position at Vetted Solutions?
This role reports to the Support Services Manager and is based in the Columbus Service Area, working M-F 8:00 am-4:30 pm with on-call and holiday rotation.
It involves providing counseling and supportive services to assist patients and families with grief, adjustment to terminal illness, and psychosocial needs.
Key responsibilities include developing and implementing care plans, offering psychosocial and grief counseling, assisting with legal and financial forms, coordinating hospice services, facilitating support groups, and participating in team meetings and quality improvement.
Additional duties include training volunteers, assisting with memorial services, and bereavement follow-up. The position requires a Master’s in Social Work, licensure (LSW/LCSW), at least one year of healthcare social work experience, and valid Indiana driver’s license.
Candidate must demonstrate excellent interpersonal, organizational, and communication skills, proficiency in Microsoft Office, and the ability to handle high-stress situations. Physical demands and safety protocols are also outlined.
