This position reports to the Manager of Support Services and is based in the Columbus Service Area, working Monday to Friday, 8:00 am to 4:30 pm, with on-call and holiday rotation.
Job Summary: Responsible for providing counseling and supportive services to patients and families coping with terminal illness, fostering adjustment and grief processing.
Responsibilities Include
Work Conditions: Mainly indoor environment, occasional lifting, and exposure to blood-borne pathogens, with physical demands including standing, walking, and reaching. Flexibility to work evenings and weekends is necessary.

- Developing and implementing care plans, offering psychosocial assessments, and providing counseling for emotional, social, and financial concerns.
- Assisting with legal document location, coordinating hospice services, facilitating support groups, and participating in team meetings and quality improvement efforts.
- Supporting bereavement follow-up, facilitating memorial services, and training volunteers.
