Post-Acute and Long-Term Care (PALTC) - Advanced Practitioner (AP) - Booneville, AR

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant responsible for independently managing comprehensive patient care in PALTC settings.

They aim to

• improve quality of care and outcomes,
• enhance patient and family experiences,
• reduce preventable hospitalizations,

• Key responsibilities include conducting assessments, diagnosing and treating illnesses, developing care plans, providing preventive services, and collaborating with facility staff and external providers. The AP ensures compliance with regulations, maintains accurate documentation, participates in quality initiatives, and promotes infection control and health education.
  
  

They must work independently, make clinical decisions, communicate effectively, utilize technology proficiently, and respond quickly in emergencies. Qualifications include a relevant degree, 2 years of experience (preferably in long-term care), board certification, and Arkansas licensure.
