Occupational Therapist

Role Overview: The Occupational Therapist provides age-appropriate rehabilitative care, addressing physical, emotional, developmental, psychosocial, and educational needs of patients in their homes. They collaborate with multidisciplinary teams to develop, implement, and revise individualized care plans, ensuring all goals are achieved.

Key Responsibilities

• Deliver direct patient care through assessments, therapeutic exercises, education, and treatments, managing patient needs from admission to discharge.
• Coordinate services with team members, develop and update care plans, and communicate patient progress to physicians.
• Educate patients and caregivers to promote independence and support recovery.
• Participate in staff training, continuing education, and quality improvement activities.
• Accurately document interventions, follow regulatory guidelines, and maintain compliance with standards.
  
  

Qualifications & Skills

• Graduate of an accredited Occupational Therapy program.
• Licenses and certifications including BLS, Occupational Therapist license, and valid driver's license with insurance.
• Minimum of one year full-time OT experience; home health experience preferred.
• Strong assessment, rehabilitative, communication, organizational, and time management skills.
• Proficiency with computer systems, home health operations, and regulatory compliance.
