- Provide effective, equitable primary care to Medicare adults, fostering long-term patient relationships.
- Conduct office visits for routine and urgent issues, administer wellness visits, and perform health assessments.
- Coordinate care with other providers, specialists, and testing facilities.
- Lead population health initiatives, ensuring preventive screenings and health maintenance.
- Assist with phone triage, patient education, outreach, and conduct home visits as needed.
- Collaborate with a multidisciplinary team within an outpatient care setting.
- Master’s or Doctorate in Nursing or NP certification with active licensure and DEA registration.
- Certification in Family, Adult-Gerontology, or Gerontological Nurse Practitioner specialties.
- Preferred experience in primary care, understanding of HCC, ICD-10 coding, and health assessments.
- Strong teamwork, communication, and patient engagement skills. Bilingual abilities are a plus.
- Full-time position, approximately 40 hours weekly.
- Compensation ranges from $95,738 to $206,206 based on experience and factors.
