Role: Home and Community Based Services Licensed Practical Nurse (LPN)

- Provide direct nursing care to acutely and chronically ill patients in their homes according to physician prescriptions
- Document patient visits thoroughly in electronic medical records and electronic visit verification systems
- Participate in care coordination, reporting patient needs and condition changes, and communicating effectively with patients, families, and healthcare team members
- Ensure compliance with infection control procedures and scope of practice defined by state laws
- Adapt to diverse home environments, ensuring high-quality patient care regardless of client background
- Unrestricted LPN licensure in the state of practice
- Current driver’s license, vehicle insurance, or access to reliable transportation
- Preferred: CPR certification, ability to work flexible hours, strong communication and organizational skills, ability to work independently, and multitask effectively
