LPN Med Care Manager

Role Overview

The LPN Medication Nurse provides licensed nursing support to ensure safe medication administration, health monitoring, and accurate clinical documentation in a senior living environment.

Key Responsibilities

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Accurately document in MAR/TAR in real-time and transcribe physician orders for care plan updates
• Monitor residents for symptoms, adverse reactions, and physical, emotional, or cognitive changes
• Report any medication refusals or adverse reactions promptly
• Maintain medication carts, perform narcotic counts, and ensure compliance with regulatory standards
• Conduct Service & Health Updates (SHUs) and support resident safety through clinical precision
  
  

Qualifications & Skills

• Active LPN license in good standing
• CPR and First Aid certifications
• Strong assessment, observation, and documentation skills
• Experience in senior care or medication management preferred
