- Perform complex clinical pharmacy services, including drug therapy management, pharmacokinetics, and multidisciplinary rounding.
- Assist healthcare providers with medication use challenges and cost containment.
- Evaluate drug therapy orders for appropriateness and monitor therapy effects.
- Document interventions, provide patient education, and counsel on drug use and storage.
- Precept pharmacy residents/students and deliver staff education presentations.
- Participate in drug use evaluation programs and contribute to Pharmacy and Therapeutics committees.
- Perform pharmacy operational duties and administrative tasks as needed.
- Doctor of Pharmacy degree or BS pharmacy with pediatric experience.
- Current Ohio pharmacy license.
- Post Graduate Year 1 (PGY1) pharmacy residency or equivalent experience; PGY2 preferred.
- Experience with adult, pediatric, or neonatal drug use, and interdepartmental collaboration.
- Proficiency in MS Office and pharmacy software like Epic Willow or Ambulatory systems.
- Minimum 3 years of relevant experience preferred.
