Primary Responsibilities
- Maintain knowledge of care management, utilization review, and regulatory updates; serve as an educational resource
- Assess physical, psychosocial, and economic needs for care transition planning
- Document and verify data to monitor interventions and outcomes
- Communicate with outpatient providers to ensure continuity of care
- Apply performance improvement and customer service principles in daily tasks
- Adhere to hospital policies, procedures, and regulatory requirements
- Perform clinical tasks within scope and ensure professional growth
- Communicate effectively in oral and written formats
- Perform additional duties as assigned
- Associate's Degree in Nursing; current RN license
- At least 1 year of experience in hospital or acute care settings
- Proficiency with Windows and typing skills
- Ability to work one Saturday quarterly
- Bachelor's Degree in Nursing (BSN)
- Experience in managed care or discharge planning
- Certified Case Manager (CCM)
- Additional case management experience
