Care Manager RN - Per Diem

Position Overview

The Care Manager RN is responsible for leading patient-centered care coordination across the healthcare continuum. Key duties include developing safe discharge plans through collaboration with patients, caregivers, and multidisciplinary teams to ensure appropriate post-discharge services and seamless care transitions. The role aims to optimize length of stay, enhance patient experience, improve throughput, and reduce readmissions by utilizing innovative, evidence-based practices and efficient resource management.

Primary Responsibilities

• Maintain knowledge of care management, utilization review, and regulatory updates; serve as an educational resource
• Assess physical, psychosocial, and economic needs for care transition planning
• Document and verify data to monitor interventions and outcomes
• Communicate with outpatient providers to ensure continuity of care
• Apply performance improvement and customer service principles in daily tasks
• Adhere to hospital policies, procedures, and regulatory requirements
• Perform clinical tasks within scope and ensure professional growth
• Communicate effectively in oral and written formats
• Perform additional duties as assigned
  
  

Qualifications & Skills

• Associate's Degree in Nursing; current RN license
• At least 1 year of experience in hospital or acute care settings
• Proficiency with Windows and typing skills
• Ability to work one Saturday quarterly
  
  

Preferred Qualifications

• Bachelor's Degree in Nursing (BSN)
• Experience in managed care or discharge planning
• Certified Case Manager (CCM)
• Additional case management experience
