The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant providing independent, comprehensive medical care in PALTC settings.
- Manage patient health through examinations, diagnoses, treatment, and preventive services.
- Work autonomously within scope, making clinical decisions and escalating as needed.
- Collaborate with facility leadership, interdisciplinary teams, and families to ensure holistic care.
- Ensure compliance with regulations, maintain accurate documentation, and participate in quality initiatives.
- Oversee infection control, educate staff and patients, and promote health and safety.
- Demonstrate strong communication, leadership, and technology skills, including EHR and telemedicine proficiency.
- Work in-person across PALTC facilities, requiring at least 2 years of experience, licensure, and board certification.
