APRN/PA - Post-Acute and Long-Term Care - Advanced Practitioner

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA who works independently in PALTC facilities, collaborating with staff and leadership to enhance patient care and outcomes.

• Manage patients through assessments, diagnoses, treatments, and preventive services, focusing on chronic and acute conditions.
• Work autonomously within scope, making clinical decisions and serving as the primary provider.
• Build collaborative relationships with facility teams, families, and external providers to coordinate care.
• Ensure compliance with regulations, maintain accurate records, and participate in quality improvement.
• Participate in infection prevention, health education, and safety protocols.
• Demonstrate strong communication, decision-making, and technology skills, with ability to work independently and respond swiftly in emergencies.
• Qualifications include a relevant degree, 2 years of experience, and board certification; long-term care experience is preferred.
