What are the responsibilities and job description for the Veterans Housing Case Manager position at Veterans Path to Hope?
This is a position that works with veterans who are homeless or recently assisted into housing through this grant (Supportive Services for Veteran Families, SSVF). The agency is headquartered in Crystal Lake and this position is with the SSVF team based out of this location. This position serves primarily as a housing case manager with homeless/at risk of becoming homeless veterans.
This work occurs in a hybrid manner, working out of various sites to meet with veterans including Lovell VA and other sites. Given that this is a mobile type of position, excellent organizational skills and a good working vehicle are required. The position provides case management with a particular focus on housing and other resources needed for the veteran's success. This position works with numerous teams, and communication skills and the ability to relate well to others are essential. The job duties include assessment of veterans who have complex needs and challenges, with the follow-up of connecting them to needed care and supports. This can be an emotionally intense position, with the highs and lows that go along with helping people change their lives for the better. Salary is discussed during interview.
Education/experience are helpful but not required, veteran or veteran connection a preferred plus. Drivers license required. Computer skills required, including comfort with Microsoft Suite and data base. Agency staff are primarily veterans or veteran-connected and you will feel welcomed!
