Actual salaries may vary depending on factors, including but not limited to skills and experience. The salary range listed is just one component of the total rewards/compensation package offered to candidates. Min = $48.32 Mid = $62.81 Max = $77.31 Summary: The Transfer Coordinator assists with processing patient admissions and transfers. The Coordinator is responsible for facilitating transfers from outside facilities and managing patient flow and bed placement. This individual occupying this position must demonstrate appropriate, diplomatic, and sensitive communication and interaction skills, all while working in a fast-paced, acute healthcare environment with multiple distractions and ever-changing priorities. Responsibilities: Provides direction, guidance, and coordination to healthcare providers for optimal, safe, effective and timely patient placements and movement and manages in-coming acute care transfers, inpatient outgoing acute care transfers and direct admissions. Communicates with referring hospital to coordinate transportation arrangements and provide an assigned beds and will assist with screening and facilitating unscheduled/urgent admissions. Connect sending and accepting physicians by a physician-to-physician call. Interacts with physicians, nursing staff and EVS to effectively communicate patient bed placements to coordinate patient throughput. Assists case management with outgoing acute transfers from the emergency department as needed. Ensures safe and timely placement of patients based on admission status, physician ordered level of care, patient’s clinical information and nursing units’ admission and discharge criteria. Maintains documentation of activities in appropriate logs and tracks and trends barriers to patient intake flow. Exhibit and model exceptional customer relationship behaviors, including telephone and personal contact. Other duties as assigned. Credentials: Essential: * BASIC LIFE SUPPORT (BLS) * REGISTERED NURSE (RN) Education: Essential: * ASSOCIATE'S DEGREE Other information: Minimum Education Required: Associates Degree Licensure required: Registered Nurse with Arizona BLS (AHA) Three years of RN experience Case Management experience preferred.
