Work Status Details: REGULAR FULL TIME | 80.00 Hours Every Two Weeks Shift: Days Pay Rate Type: Hourly Location: Behavioral Health Listed is the base hiring salary range offered for this position. Actual salaries may vary depending on factors, including but not limited to skills and experience. The salary range listed is just one component of the total rewards/compensation package offered to candidates. Min = $20.95 Mid = $26.19 Max = $31.43 Summary: The Case Management Assistant serves as extender for case management team and professional staff, assisting/facilitating in non-complicated discharge arrangements. Responsibilities: Completes social function assessment. Assists with discharge arrangements/transfers. Arranges for transportation of patients REQUIRED:

• 2 years related experience
• High School Equivalency Diploma
