What are the responsibilities and job description for the Housing Specialist position at Veterans & Community Housing Coalition?
This position involves working with the SSVF program for homeless veterans and also the Medicaid Waiver Program.
SSVF Program Responsibilities:
· Establish and maintain collaborative working relationships with community landlords and leasing offices.
· Research and identify suitable housing options for SSVF clients and communicate these to the clients.
· Maintain precise and accurate case notes in the HMIS database of housing services provided to the client.
· Maintain and update internal tracking systems.
· Participate in regular staff meetings and SSVF required meetings and trainings.
· Complete rent reasonable comparisons and Habitability Inspections.
· Collaborate with other local VA Housing Programs (including but not limited to HUD VASH, other SSVF grantees etc.), local housing authorities, housing programs, and local community agencies to achieve successful rapid rehousing or prevention of homelessness.
· Transport clients without transportation to look at housing options.
· Adhere to the policies and procedures outlined in the SSVF Program Guide including the VA Housing First Model and Rapid Rehousing.
· Other duties and responsibilities to achieve program mission.
Medicaid Waiver Program Responsibilities:
· Provide tenancy sustaining services to support housing stability for Medicaid eligible individuals.
· Complete required screenings and assessments to determine eligibility and service needs.
· Coordinate rental and utility assistance in alignment with program guidelines.
· Manage referrals and collaborate with community and health care partners.
· Maintain accurate documentation and submit required information for billing and reporting.
· Any other duties assigned by your supervisor.
Pay: $43,000.00 per year
