Duties
- Assist client with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support.
- Monitor patient health by observing changes in condition to prevent fall risk
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Support medication reminders following HIPAA guidelines and care plans to ensure safety and compliance
- Maintain a clean and safe environment through regular cleaning tasks, laundry assistance, and organization of personal spaces
- Provide emotional support and companionship to reduce feelings of loneliness or confusion, especially for individuals with dementia or Alzheimer’s disease
- Document care activities thoroughly in compliance with healthcare regulations and assist with care plan updates as needed
Skills
- Proven experience in home & community care, assisted living, nursing homes, or group homes setting
- Knowledge of patient observation techniques and patient monitoring to identify changes in condition promptly
- Certification or training in CPR, First Aid, HIPAA compliance, and medication administration preferred
- Ability to perform heavy lifting safely and operate assistive devices such as mobility aids
- Strong communication skills for effective interaction with clients, families, and healthcare teams
- Familiarity with care plans, vital signs measurement, hospice care principles, and long-term care practices
