Rate: $29/hour
Shift: Part-Time
Schedule: Mondays and Tuesday, 8:30 am- 4:30 pm

Susquehanna Nursing & Rehabilitation Center/Golden Days Adult Day Healthcare Program is looking to add a Licensed Practical Nurse (LPN) to its Adult Day Care team. In this role, you’ll provide nursing care and support to registrants while helping ensure a safe, engaging, and compassionate environment. RESPONSIBILITIES:
- Provide nursing care to registrants in accordance with established policies and procedures
- Supervise nurse aides in providing care, meals, and activities
- Monitor registrant health and well-being and document care accurately and timely
- Communicate changes in condition to responsible parties and maintain appropriate documentation
- Assist with coordinating transportation for registrants as needed
- Promote a safe, supportive, and person-centered environment
- Collaborate with interdisciplinary team members to support quality care
- Perform additional duties as assigned
REQUIREMENTS:
- Current NYS Licensed Practical Nurse (LPN) license required
- Strong communication and interpersonal skills
- Ability to supervise and support nursing assistants and other staff
- Strong organizational and documentation skills
- Ability to provide compassionate, resident-centered care
- Professional, dependable, and able to work effectively as part of a team
