Director of Clinical Services (RN) – Home Health

Vertus HealthCare is a growing home health agency dedicated to delivering a higher standard of patient-centered care. We are seeking an experienced and compassionate Director of Clinical Services (RN) to join our leadership team and help drive clinical excellence across our organization.

Position Summary:

The Director of Clinical Services is responsible for the overall direction, coordination, and oversight of all clinical operations. This leadership role ensures high-quality patient care, regulatory compliance, and effective coordination among clinical and administrative teams.

Clinical Leadership & Oversight:

• Develop, implement, and manage clinical programs and operations
• Supervise and support clinical staff, including RNs, LPNs, therapists, and home health aides
• Review and approve patient assessments, plans of care, and clinical documentation
• Ensure compliance with all federal, state, and Medicare home health regulations
  
  
  

Care Coordination & Quality:

• Conduct case conferences and participate in interdisciplinary team meetings
• Monitor patient outcomes, staff productivity, and caseload distribution
• Lead quality improvement initiatives (QAPI) and ensure documentation accuracy
• Provide field supervision and performance evaluations as needed
  
  
  

Operations & Collaboration:

• Collaborate with intake, scheduling, and administrative teams to ensure efficient workflows
• Assist with hiring, onboarding, and training clinical staff
• Serve as a liaison between clinicians, physicians, patients, and families
• Ensure adherence to PDGM guidelines and care coordination standards
  
  
  

Requirements:

• Graduate of an accredited School of Nursing
• Active Florida Registered Nurse (RN) license in good standing
• Minimum 2 years of experience as an RN
• At least 1 year of management experience in home health care
• Clinical leadership experience in a Medicare-certified home health agency
• Strong knowledge of:
• OASIS documentation
• PDGM (Patient-Driven Groupings Model)
• QAPI (Quality Assurance and Performance Improvement)
• Excellent leadership, communication, and organizational skills
• Passion for delivering high-quality, patient-centered care in a home setting
