Position Summary

This position description has been prepared to assist in evaluating duties, responsibilities, and skills of the position. It is not intended as a complete list of specific responsibilities and duties, nor is it intended to limit duties to those listed.

This position provides a variety of case management and other direct services in programs for persons with diagnosis of mental illness and individuals experiencing a mental health crisis. Employee exercises independent judgment within established guidelines.

Essential Duties and Responsibilities

  • Coordinates and assists individuals experiencing a mental health crisis.
  • Assists and collaborates with partners with assessments of individuals placed under a Chapter 51 emergency detention.
  • Prepares written and oral testimony for Circuit Court for all Mental Health cases, which may include but is not limited to: Mental Health Court Commitments, 3 party petitions.
  • Prepares court petitions and reviews for Circuit Court for all Mental Health cases, which may include but is not limited to: Mental Health Commitments, 3 party petitions.
  • Assists with Guardianships and Protective Placements, Ch. 54/55.
  • Prepares court petitions, reports, comprehensive evaluations for adults with mental health diagnosis under Chapter 54/55 of the Wisconsin Statutes.
  • Complete all other necessary court reports, follow-up correspondence, and prepare necessary reports to the department or state.
  • Provides and/or arranges for transportation of consumer for mandatory court appearances.
  • Provides short term and ongoing case management of identified consumers with mental health and/or AODA concerns.
  • Arranges for placement services within community-based resources such as Adult Family Homes and Community Based Residential Facilities.
  • Coordinates services for clients with mental illness and/or AODA issues who are living in a variety of community settings.
  • Provides intake and assessment of individuals who are seeking AODA treatment options and funding options available. Coordinate and assist with individuals determined eligible with arranging for treatment options.
  • Coordinates and acts as a liaison between the county and contracted provider of the Community Support Program (CSP). Will oversee program integrity and enrolled consumers in the program. As well as review claims/expenses for this program and individuals being served in CSP.
  • Responsible for being a back-up social worker for: conducting investigations of allegations of physical, emotional, financial, and sexual abuse, as well as allegations of neglect or self-neglect of vulnerable adults and the elderly with mental health concerns.
  • Reviews and follows policies and procedures to ensure compliance with Federal, State, and County laws and regulations.
  • Writes internal policies/procedures when appropriate for programs in the unit.
  • Contacts State offices and other professionals to maintain compliance with programs.
  • Speaks to community groups regarding the services available through the Long-Term Support Unit.
  • Responsible for services through the Wisconsin Children’s Long-Term Support Program (CLTS) and Children’s Community Options Program, which is defined by the Federal and State guidelines and requirements.
  • Conducts intake and assessments for both the Children’s Long-Term Support Program and Children’s Community Options Program.
  • Determines eligibility for both the Children’s Long-Term Support Program and Children’s Community Options Program.
  • Coordinates enrollment, develops, implements, and reviews service plan, for both the Children’s Long-Term Support Program and Children’s Community Options Program.
  • Provides on-going monitoring/case management to recipients (eligible children) and their families.
  • Coordinates and oversees services for children in the Children’s Long-Term Support Program and Children’s Community Options Program. Acts as liaison between potential contracted agency and county.
  • Provides expertise in navigating the early intervention system using family centered practices, linking families to community resources, fostering strong family professional partnerships.
  • Service coordinators coordinate the delivery of all services across agency lines and connect children and their families with services needed as identified in the ISP.
  • Ensure families are informed, involved, and consulted during their child’s enrollment.
  • Coordinating and monitoring the delivery of an array of services to address each child’s unique needs and circumstances as outlined in the Individual Service Plan.
  • Ensures state and federal compliance with all the Long-Term support programs.
  • Maintains individual consumer files. Maintains documentation and case notes, for the ongoing case management requirements. Maintains &updates all files, reviews cases as mandated and necessary for compliance.
  • Reviews monthly expenses/claims, prepares consumer information for data entry in the Program Participation System (PPS) and any county software.
  • Authorizes services for consumers based upon assessed need within the limits of available funds, following allowable services guidelines, and following state and federal guidelines for the programs.
  • Assists with rotational on call for after hours for CH. 51 and Ch. 55’s.
  • And all other duties assigned by the unit supervisor.

  • Ability to communicate with the public and Vernon County staff in-person and on the phone.
  • May travel to other Vernon County locations.
  • Occasionally may handle stressful situation(s) and short deadlines.
  • Involves minimal potential and/or direct exposure to blood or bodily fluids.

  • Capable of lifting and carrying light to moderate office supplies.

  • Involves potential threat of physical attack or injury from consumers/animals in unusual situations.
  • Involves potential and/or direct exposure to blood or body fluids.

Education and Training Requirements

  • Graduation from an accredited college or university with a minimum Bachelor degree in a human service discipline, such as Social Work, Sociology, or Psychology. Minimum of one year of county social work experience is preferred. MUST HAVE current Wisconsin Social Work License meeting the requirements specified under Wis. Statute 457 or ability to become license or certified.
  • Possession of a valid driver’s license and reliable transportation with adequate auto insurance to meet minimum County requirements, or access to reliable transportation for city and rural travel.
