What You’ll Do:
- Deliver exceptional PT services to evaluate and treat patients in their homes
- Work closely with a compassionate care team
- Help patients regain independence and quality of life
Your Role:
- Evaluate and treat patients in the home
- Help improve mobility, strength, and safety
- Document using EMR (Kantime a plus)
What You’ll Need:
- Active CO PT license
- 1 year in acute care or home health (required)
- Medicare home health experience is highly preferred
- Passion for patient-centered care
- EMR experience
