The Center at Cordera is seeking a compassionate and organized Case Manager to join our interdisciplinary team. The Case Manager is responsible for coordinating resident care, discharge planning, and psychosocial services to ensure residents receive individualized support throughout their stay. This role works closely with residents, families, physicians, hospitals, community agencies, and the care team to facilitate successful transitions and enhance quality of life.
Essential Responsibilities
- Complete psychosocial assessments and maintain resident social histories.
- Coordinate discharge planning, including communication with hospitals, families, physicians, insurance providers, and community agencies.
- Assist residents and families with discharge needs, including home health, hospice, durable medical equipment, transportation, and community resources.
- Provide education regarding Medicare, Medicaid, insurance benefits, advance directives, and available financial assistance programs.
- Participate in interdisciplinary care conferences, resident care planning, and MDS meetings.
- Advocate for resident rights and assist in resolving resident and family concerns.
- Coordinate referrals to community resources and support services.
- Maintain accurate and timely documentation in PointClickCare and resident medical records.
- Collaborate with nursing, therapy, admissions, dietary, activities, and other departments to ensure continuity of care.
- Monitor discharge goals and barriers, proactively working to facilitate safe and timely transitions.
- Assist with regulatory compliance, survey readiness, and quality improvement initiatives.
- Maintain knowledge of federal, state, and facility regulations related to resident care and discharge planning.
- Perform other duties as assigned.

Minimum Qualifications
- Bachelor's degree in Social Work or a related human services field preferred.
- Minimum one (1) year of experience in skilled nursing, long-term care, or healthcare case management.
- Experience with discharge planning and community resources preferred.
- PointClickCare experience preferred.
- Knowledge of Medicare, Medicaid, resident rights, and long-term care regulations.
- Valid driver's license with an acceptable driving record if transportation or community visits are required.
Salary : $20 - $25
