Job Description Summary ‎ **Candidates in the Columbus region strongly preferred** The Case Manager plays a vital role in delivering client-centered services that support patients across the continuum of care. This position partners with patients and their families to coordinate healthcare and connect them with appropriate home- and community-based services.
