What are the responsibilities and job description for the Whole Person Care Community Services Coordinator position at Ventura County?
POSITION:
The Ambulatory Care Division of the Ventura County Health Care Agency (HCA) is currently looking to fill nine (9) Community Services Coordinator positions in the Whole Person Care team. The Whole Person Care team implements the CalAIM Enhanced Care Management and Community Supports programs which works to improve care for people with the most complex needs including people with behavioral health needs such as serious mental illness, emotional disturbance, and/or substance use disorder; seniors and people living with disabilities, people experiencing homelessness who also have complex physical or behavioral health needs, people transitioning from jail or prison back to the community, children and youth with complex medical conditions and/or in foster care. CalAIM Community Supports substitutes new clinical and non-clinical services for traditionally covered services like care in a nursing home or hospital to meet needs in new, more patient-focused ways. Local investments include housing supports, short-term recovery supports, and independent living supports.
Working as part of a multi-disciplinary team, the Community Services Coordinator provides intensive, person-centered, field-based care management services including assessments, informed consent for services; conducts care planning, goal setting, and motivational interviewing, helps to navigate health, behavioral health, and social services, coordinates with other providers, and documents notes, and services provided in the electronic health record.
EDUCATION/BILINGUAL INCENTIVES: Incumbents may be eligible for educational incentive of 2.5%, 3.5%, or 5% based on completion of an Associate's, Bachelor's, or Master's degree that is not required for the classification. Incumbents may also be eligible for bilingual incentive depending upon operational need and certification of skill.
NEW HIRE/RETENTION/REFERRAL INCENTIVES: New hires may be eligible for a one-time New Hire Incentive of $1,500, subject to the applicable section of the SEIU 721 MOA 2022-2025, Article 38.

Community Services Coordinator is represented by the Services Employees International Union (SEIU) and is not eligible for overtime compensation.

- Independently, and as part of a coordinated care team assesses and develops effective plans of care in response to multiple and complex patient health, behavioral health, housing and social service needs; helps patients identify their unique strengths in addressing difficult health and social issues.
- Plans, organizes and coordinates assigned caseload and staff activities including case management, records management, and community outreach services and projects.
- Serves as liaison with representatives from state, County, and local agencies, school districts, courts and the community to identify and resolve problems/needs.
- Interprets and implements federal, state and County laws, regulations, and policies to ensure adherence to state and County program and fiscal guidelines.
- Prepares a variety of reports, and correspondence; keeps appropriate records, documenting patient encounters and progress notes consistent with agency requirements.
- May conduct or evaluate needs assessments, initial and follow-up screening and social histories of patients referred to the program.
- May transport patients to medical, behavioral health, and social services appointments at County and community facilities when required.
- Performs other related duties as required.

EDUCATION, TRAINING, and EXPERIENCE:
Considerable experience in social work, community health, mental health or education which has led to the acquisition of the required knowledge and abilities.
The required knowledge and abilities also can be obtained by four (4) years of experience in social work, community health, mental health or education, including responsibility for program development and implementation.
NECESSARY SPECIAL REQUIREMENTS:
- Must possess and maintain a valid California driver license
- All positions will require some weekends and evenings.
- At least one (1) year of experience in community outreach and education.

KNOWLEDGE, SKILLS, and ABILITIES:
Thorough knowledge of: community structure and resources; group and family dynamics; County roads and buildings; problems, needs and behavioral patterns of the population served, countywide systems of services and supports, medical, social, behavioral and housing to serve at-risk populations; and specialized topics for group presentation.
Working ability to: follow instructions; provide services in a field-based environment; exercise sound judgement; develop trusting and effective working relationships with a diverse patient population including persons with health, mental health and substance use issues, many who present with challenging behaviors; transport patients when required; write timely, accurate case chronologies, field service documentation in the electronic health record, and other reports regarding patients; administer various health/behavioral health screening tests such as social needs, patient activation measures, substance use assessments, depression screening tools, and suicide risk questionnaires; develop educational programs and evaluation plans; coordinate activities among various groups; plan and organize work of others; conduct classroom training or educational workshops; communicate effectively orally and in writing.
Skill in: motivational interviewing, de-escalation, non-violent crisis intervention, and patient education.
