Under the clinical supervision of a physician, Advanced Practice Provider (APP), or Registered Nurse, the Licensed Vocational Nurse II assigned to the Surgical Subspecialty Clinic at Ventura County Ambulatory Care provides skilled vocational nursing care in a fast-paced outpatient surgical subspecialty environment. This includes supporting a high volume of specialty consultations, in-house procedures, and assisting in pre- and post-operative care orders.  The Licensed Vocational Nurse II (LVN II) assists physicians and clinical staff in delivering patient care within the scope of vocational nursing practice in the State of California. 

Distinguishing Characteristics:
The Licensed Vocational Nurse II performs a broad range of clinical support duties under routine supervision, assisting physicians, advanced practice providers, and Registered Nurses in delivering patient care. Incumbents are expected to work within the scope of their licensure and in accordance with established clinic procedures and protocols.

EDUCATIONAL/BILINGUAL INCENTIVE: Incumbents may be eligible to receive an educational incentive of either 2.5%, for an associate's degree, 3.5%, for a bachelor's degree, or 5% for a master's degree that is not required to work in the classification. Incumbents may also be eligible for bilingual incentive pay depending upon operational needs and certification skills.
 
AGENCY/DEPARTMENT: Health Care Agency – Ambulatory Care

Primary Care Clinic Licensed Vocational Nurse II are represented by the California Nurses' Association (CNA) and is eligible for overtime compensation. Salary placement will be determined according to the current CNA memorandum of agreement.
 
The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-Term), Per Diem Pool, Intermittent, and Extra-Help vacancies within the Health Care Agency for these positions only

Duties may include but are not limited to the following:

  • Performs patient care functions and initiates appropriate nursing interventions as outlined in standards of care and the individual patient care plan in support of professional staff.
  • Actively participates in assigned patient care team duties and activities, including pre-visit planning, identifying gaps in care, health maintenance needs, and team huddles.
  • Participates in schedule management procedures to support optimization of schedules.
  • Administers medications as ordered by the physician or APP.
  • Safeguards patient’s health and safety by reporting abuse and neglect, maintaining patient confidentiality, and maintaining current skill and knowledge of safe and competent patient care.
  • May perform Clinical Laboratory Improvement Amendments (CLIA)-waivered testing after obtaining and maintaining annual competency.
  • Performs intake, including vitals and patient measurements, such as height weight, blood pressure, temperature, pulse, and oxygen saturation.
  • Performs routine patient care procedures, such as venipuncture, dressing changes, throat swabs, and hearing tests. 
  • Prepares patient and exam room for exam, treatment, and/or procedures and serves as chaperone or assists as needed; ensures required documentation and consent forms are completed.
  • Monitor patients before, during, and after procedures and promptly report changes in condition to supervising clinical staff.
  • Ensures appropriate documentation in the electronic medical record.
  • Collects and prepares specimens, including blood, urine, stool, and sputum.
  • Coordinates patient care by following up on orders, including referrals to ancillary and specialty services and assists with authorizations for procedures, surgeries, diagnostic testing and pharmaceuticals. 
  • Maintains clean and safe patient areas. Demonstrates adherence to and observes all safety policies and procedures, inclusive of infection control rules and regulations. 
  • Answers telephones and takes messages, reviews portal messages and routes as appropriate, and completes assigned tasks to ensure patient needs are addressed. 
  • Demonstrates cultural sensitivity and competence with patients. 
  • Communicate with diverse patients regarding physician instructions. 
  • Reports patient safety incidents to supervisor and via the patient safety reporting software. 
  • Participates in performance improvement processes; and provide clinical and technical direction.
  • May perform or provide preoperative preparation, including pre- and post-procedural education, including wound care instructions, medication guidance, and follow-up care.
  • Performs catheter insertions and irrigations.
  • May insert, maintain, and discontinue non-medicated intravenous therapy,
  • Administers first aid or cardiopulmonary resuscitation in emergency situations until advanced medical support is available.
  • Performs duties within scope of vocational nursing practice in the State of California.

EDUCATION, TRAINING, and EXPERIENCE:
Requires one (1) year of full-time licensed vocational nursing experience in a health care setting, and completion of the Primary Care Clinic Licensed Vocational Nurse I competencies established by the Ambulatory Care Nursing Administrator.

Necessary Special Requirements:

  • Possession of a current valid license as a Vocational Nurse issued by the State of California.  
  • Experience in an outpatient surgical subspecialty setting providing pre- and post-procedure patient care and clinical support.
  • Must have a current and maintain a valid Basic Life Support (BLS/CPR) certification by first day of employment.

Knowledge, Skills, and Abilities:

Knowledge of: the principles and practices of vocational nursing; medical terminology, routines, and clinical equipment commonly used in an outpatient surgical or specialty clinic; infection control and sterile technique; medication administration procedures; CLIA-waived laboratory testing; electronic health record documentation standards; and patient privacy, abuse/neglect reporting requirements, and confidentiality policies.

Skill in: performing patient intake and clinical support activities including obtaining and recording vital signs and patient measurements; assisting physicians and advanced practice providers during examinations and minor procedures; performing routine clinical procedures such as dressing changes, specimen collection, venipuncture, electrocardiograms, and CLIA-waived testing; and preparing and maintaining exam and procedure rooms with appropriate equipment and supplies.

Ability to: administer medications and injections as ordered by physicians or advanced practice providers; follow established clinic procedures and infection control protocols; observe and report changes in patient condition to appropriate clinical staff; provide basic patient instructions regarding procedures and follow-up care; maintain accurate documentation in the electronic health record; maintain patient confidentiality; establish effective working relationships with patients, families, and healthcare staff; and communicate effectively both orally and in writing.
